Log triage

Declares Agent/LogTriage standing: triages a red log incident and drafts its GitHub issue.

• Purpose: Triages a red log incident — a distinct error fingerprint seen in production — naming the probable defect and the owning repository, and drafts the issue.
• Triggers: A new LogIncident — a fingerprint the in-cluster log watcher reports to the portal's ingest endpoint (armed only when LogWatch:IngestToken is set)
• Runs on: every portal whose log watcher is configured
• Wired: yes
• Queue path: Admin/_LogIncident
• Thread namespace: {incident}/_Thread
• Note: Each incident's thread hangs off the incident node itself (`Admin/_LogIncident/{fingerprint}/_Thread/…`). Open items are not counted here: the incidents live in the Admin system partition.
